Summary: We identified cardiac transthyretin/leukocyte chemotactic factor (LECT) 2 double amyloidosis as the cause for heart failure in a Caucasian male presenting with dyspnoea. To date, no report exists about cardiac ALECT2 manifestation. In addition to transthyretin, cardiac LECT2 amyloidosis was identified by immunohistochemistry (IHC), and confirmed by matrix-assisted laser desorption/ionization imaging mass spectrometry (MALDI-IMS). Furthermore, genomic examinations on DNA showed homozygosity for the G allele. Our findings suggest that cardiac ALECT2 might be underdiagnosed, especially in presence of other more common types of amyloidosis and should be added to the list of possible causes of a primary cardiac amyloidosis.
