The Charles Bonnet syndrome consists of complex visual allucinations occurring in elderly patients with visual disturbances, in the absence of psychosis, degenerative neurologic diseases or drug abuse. A 85 year-old men presented with a secondary partial epilepsy due to a right occipital lobe infarct. He was neither a psychotic nor a drug abuser He began treatment with Topiramate 25 mg/die and, two days later, complained of visual illusions and complex allucinations occurring many times a day which worsened with dose increase to 50 mg/die. During the episodes EEG did not show epileptiform activity. After the drug was stopped the disturbance recovered within 48 hours.
