Essential thrombocytosis (ET) is a haematological disease caused by abnormal megakaryocyte proliferation that could lead to a worsening of mood symptoms. We describe a woman with ET comorbid with bipolar disorder. The initiation of therapy with hydroxy‐oncocarbide improved psychiatric symptoms. The presence of medical conditions in psychiatric presentations should always be considered.
