Reviewing the histopathologic findings of a series of 518 radical nephrectomies, small cell carcinomas of the kidney were identified in 4 patients, 2 men and 2 women, ranging in age between 35 and 68 years. Immunohistochemical studies were performed using monoclonal antibodies against Chromogranin A and Cytokeratin with ABC procedure, and they revealed positivity for chromogranin A in the majority of the cells in one case, supporting the neuroendocrine lineage of these tumors, presumably originating from a totipotential stem cell. Clinically these neoplasms were uniformly of high stage at presentation and aggressive, since all patients died within 16 months of surgery.
