A case of a 27-year-old male with renal colic and a silent left kidney at IVP is presented; a renal mass was evident both at sonography and CT scan and a nephroureterectomy was performed. Pathology revealed polypoid tumours of the pyelocalyceal system and the ureter with both epithelial and sarcomatous elements. The um resembled normal transitional cells, while the sarcomatous component showed undifferentiated cells and rhabdomyoblastic elements, fulfilling criteria of 'mesonephric adenosarcoma'. This neoplasm has only recently been characterized and it seems that only 2 other cases with satisfactory histologic documentation, have been reported.
