A finding of positive margins after radical prostatectomy for prostatic adenocarcinoma is a possible occurrence which influences the mode of treatment and prognosis of the disease. With the aim of identifying the factors which determine the state of the surgical margins we evaluated the clinical and pathological data from 80 patients who underwent extracapsular prostatectomy from January 1993 to December 1994. Of the 80 patients 33 (41.25%) had positive margins of resection. These cases were directly correlaled with tumor volume as well as pathological stage and grade, while no statistically significant correlation was shown with the level of preoperative PSA. The clinical determination of these factors can lead to the identification of patients at high risk of positive margins, for whom neoadjuvant hormonal treatment may be indicated with the aim of reducing such occurrences.
